The U.S. Department of Housing and Urban Development (HUD) requires the state to prepare an annual Ohio Consolidated Plan to govern administration of federal Community Development Block Grant (CDBG), Home Partnerships Investment Program (HOME), and Emergency Solutions Grant (ESG) resources.

As part of the planning process, the Ohio Development Services Agency (ODSA) has prepared three separate surveys to determine the state’s housing, homeless and community development needs for the 5 Year Consolidated Plan covering the period of PY 2020 – 2024.

The surveys are open to the public and will remain anonymous. You may complete all three surveys even if you do not administer a program in one subject. ODSA intends to keep the plan to keep the surveys open until at least the start of the Program Advisory Committee meetings in late October.
